Things to Remember for 2009-2010


REMEMBER: To provide benefit coverage for your spouse or any eligible dependent children, it is REQUIRED that you provide the Social Security # for them this year.


Mansfield ISD Cafeteria plan year is November 1, 2009 through October 31, 2010.

Flexible Spending Accounts - MGM is the Third Party Administrator (TPA) for Mansfield ISD for plan year November 1, 2009 through October 31, 2010. Direct deposit is available. The maximum annual contribution for medical reimbursement is $2,400/plan year. The maximum annual contribution for dependent care reimbursement is $5,000/plan year. Important Reminder: The same IRS125 Plan that allows you to participate in this plan also requires that you forfeit any unused money left in your account at the end of the Plan year or at your termination. This is the "USE IT OR LOSE IT" provision of the IRS Code. It is very important that you estimate your expenses carefully and conservatively. YOU MUST RE-ENROLL THIS YEAR FOR THE FLEXIBLE SPENDING ACCOUNT. Benefits will not “roll-over” from last year.

Important Reminder: You will be receiving a “NEW” MGM Flex Card after the completion of the plan year enrollment. Please make sure your mailing address is correct!!

Dental - Guardian is the dental provider effective 12.01.2009. NEW! This year Guardian is offering a PPO, a Value Plan, and DHMO dental plan option. Please review the plan information under “Benefits and Forms/Dental” before making your selection.

Vision - Block Vision is the vision plan provider effective 12.01.2009. View more plan information and rates under “Benefits & Forms/Vision”.

Disability - Unum is the disability plan provider effective 12.01.2009. Employees can enroll for a monthly benefit amount up to 66 2/3% of gross annual salary on a guarantee issue basis. *Guarantee issue coverage is subject to a12-month-pre-existing condition exclusion. Cancer - American Public Life (APL) - APL is a group cancer plan that is Guarantee Issue …this means NO HEALTH QUESTIONS ARE REQUIRED to enroll (subject to 12-month pre-existing condition exclusions). View a complete brochure, plan summary and rates under the “Benefits & Forms/Cancer” link

Voluntary Life - Fort Dearborn - Employees enrolling the plan as late entrants (adding plan or increasing benefits amounts) are required complete an evidence of insurability form to be considered for coverage under this plan (a physical may be required). Hospital Indemnity Plan - Cigna is the HIP plan effective 12.01.2009. This plan is employer paid for eligible employees who elect to waive participation in the school district’s health insurance plan. Completion of an Enrollment form and beneficiary designation is required.

Supplemental Benefits Plan year is December 1, 2009 to November 30, 2010.

Mansfield ISD Supplemental Benefit plans for 2009-2010 plan years include: CIGNA HIP plan, Guardian PPO, Value and DHMO Dental, Block Vision, Unum Select Educator Disability, American Public Life Cancer, Fort Dearborn Term Life and AD&D, and MGM Flexible Spending Accounts.
