Highlights of 2011 - 2012 Supplemental Plans
Annual enrollment for ALL Benefits will be August 1 to August 31, 2011 All benefits become effective on 9/1/2011.
Supplemental Insurances require employees to work a
minimum of 20 hours per week to be eligible.
Mansfield ISD Cafeteria plan year is September 1, 2011 through August 31, 2012.
- Flexible Spending Accounts - NEW! The new administrator for the Flexible Spending Account is: National Benefits Services (NBS) effective 09/01/2011. Your FSA elections for the current year WILL NOT automatically re-enroll to the new plan year. You will have to re-enroll for the 2011-2012 plan year, even if you are electing the same amount. ** To review the status of your current MGM FSA account via the HUB, select “Check FSA” on the bar above. If you have questions call MGM at 800.833.4028. “NBS” plan year eligible expenses must be incurred in the plan year of 09/1/2011 through 8/31/2012. Remember that over-the-counter medicines or drugs will no longer be eligible for reimbursement under the FSA plan without a doctor’s prescription. IRS may audit the flexible spending accounts so KEEP YOUR RECEIPTS!
- The MGM Card that you currently have will expire on 8/31/2011. The new “NBS” Flex card will be sent to your home address 2 to 3 weeks after the first of September 2011, if you have updated your address in the system. You will receive one (1) card, but you may order a card for your spouse or child over age 18 at no cost to you, by contacting NBS once you have received YOUR employee Flex Card. There is NO MONTHLY COST for the NBS Flex card to the employee. For replacement of a misplaced/lost card the fee is $10. For participants in the Medical Reimbursement account, the new NBS Flex Card is valid for 3 years, so keep your Flex card! This card will be re-loaded next year with your new FSA election amount. If you prefer to mail in your claim for reimbursement, or if the vendor does not accept the NBS Flex Card, claim forms are available on-line. - IMPORTANT! IRS SECTION 125 CAFETERIA PLAN RULE - All benefit elections will remain in effect and cannot be revoked or changed during the plan year unless you have one of the following changes in family status: Marriage, Divorce, Birth, Adoption, Death, Termination or change in employee or spouse’s employment, Change in eligibility status of a dependent. NOTE: Documentation from an independent third-party verifying that you have experienced the indicated change within 30 days of the event is required. Contact the MISD Benefits Coordinator, for questions or to make any changes. If you miss the 30 day window, nothing can be changed until the next open enrollment.
- TRS-ActiveCare - Effective 9/1/2011 TRS ActiveCare/BCBS will have an increase in premiums and changes to benefits. New ID cards will be mailed out for ALL enrollees and MEDCO will issue new ID drug cards.
- Guardian Dental - Offers 3 options for you to choose from. For additional questions you can review the plan information under “Benefits & Forms / Dental” or review the video in the “Quick Links” section of your benefits website before making your selection.
a. The Network Access dental plan (NAP) covers in-network benefit percentages to 80% for preventive, 50% for Basic and 25% for Major Services.
b. The Value PPO dental plan has a 12-Month waiting period for Major and Ortho services that applies for all NEW enrollees (does not apply if switching from the NAP or DHMO plan).
c. The DHMO (Prepaid Plan) dental plan has no waiting periods, claim forms or deductibles. Orthodontia is covered for Children and Adults. Provider selection is required at enrollment and no benefits will be paid if you see a provider that is not pre-approved through Guardian’s Managed DentalGuard Plan.
- Block Vision - Block is the vision plan provider effective 09/01/2011. To view more plan information and rates, look under "Benefits & Forms / Vision" section.
- American Public Life (APL) Accident Plan - APL Accident is being offered effective 9/01/2011 to you and your family. View more plan information and rates under "Benefits & Forms/Accident."
- UNUM Disability - Is the disability plan provider effective 9/01/2011. Coverage is subject to a 12-month-pre-existing condition exclusion. View more plan information and rates under "Benefits & Forms/Disability."
